To complete this form, please have your 'Tis the Season Pass email confirmation or your 'Tis the Season Pass printed pdf confirmation pages handy, so you can input the barcode numbers of each season pass. You will be asked for the name and phone number of the purchaser of the pass, in case we need to look up the order. You also will be asked to provide personal details for the pass holder including name and mailing address, and we require you to upload a pass holder photo, so we can print and mail your plastic ID-style permanent 'Tis the Season Pass card.
